I have been reading "The Silent Knife" book & as a result have even more questions I think . DH has never seemed particularly interested in listening to me spout on about Liam's birth or the next one so you ladies are it .

The book is quite dated now but some of the info is still relevant & one of the main points I have gathered is that you have more chance of a successful VBAC when you have no intervention - ranging from rupture of your membranes right through to an epidural.

For those that have had VBAC's did you have an epidural? What did you do for pain relief, if any?

I had an epi with Liam (obviously ) but it was done hours before I went into theatre. I realise that each labour & birth is different but at one stage when the epi wore off, I found the pain excuriating & I'm just not sure how long I could cope with that kind of pain again.

I apologise for my ramblings but as I said, there's not really anyone else I can discuss this with. I am determined to have a VBAC but I don't think my head is quite in the right place just yet. Lucky I have plenty more time...

I had a c/s with DD1. I remember feeling so much relief with the epidural I had for that, that I decided early on that I wanted an epi for my VBAC (DD2). I went into labour slowly with DD2, her head wasn't quite in position to break my waters but she was okay to VB. I had ARM (artificial rupture) while in "early labour" (had been over 24hrs of contractions by then) and soon after went ahead with the epi - didn't even try the gas first. Anyway, my labour progressed pretty quick after that and I had to push without any feeling - and consequently took a while to do that. I had to have an episiotomy but otherwise did ok. My conclusion from that birth was that having the epidural was actually unhelpful when I still had work to do.

DD3 was born with gas, fentynal and a tear.

DD4 was born with just gas - not my choice but it went too quick for any decent pain relief LOL. But... I gotta admit that I had so much more feeling of what I was, and needed to be, doing.

With my VBAC I had to be at the hospital pretty much straight away as I had some complications 10 days earlier that nearly resulted in another cs then. So my whole labour was on the bed with up in stirrups. Labour started at 1am, was at hospital by 2.30am.  Within 2 hours of starting my contractions were 2 mins apart and nearly a minute long and very painful as I was not able to move so I gas. Found it didn't seem to do a lot until the contraction had nearly finished (but thats probably cos I kept taking it away from my face to have fresh air). Ended up having an epi at 7am and was fully dialated by 8am but Joel was too high and wasn't coming down at all (10 days early). Pushed for 2.5hrs then he started to get distressed (and I wasn't lol) so it was either another cs or forceps. Chose forceps, had an episiotomy and 2nd degree internal tear but I was up and about 6 hours later and home within 24 hours so much better than the 6 days with no1.

My midwife knew I really didn't want a cs and while not a lovely easy birth was still for me a better birth than with no1. (there ended up being 10 people in the room as all the drs had students and so did the midwife, and they all asked if it was ok for the students. I figured they had to learn about a forceps birth anyway so they might as well be there! The student OB was the one who delived Joel and he didn't have a mark on him.)

My labour with Kyle started at 3pm with contractions 3-5 mins apart lasting 30 secs, at about 4.30ish my waters broke (just my hind waters though) and then contractions got a bit longer and more intense but still the same interval. Whilst at home I walked around, then straddled a chair and got Dh to rub my back between contractions (didn't want to be touched during them). After about 5 hours at home we headed into the hospital as I was getting worried about the car journey and discomfort (40min drive). Laboured for another 4 hours at the hospital with contractions 2 mins apart and lasting 1min, used walking, straddling a chair again, tried going in the shower and then had some gas (which did jack sh*t). I then demanded an epidural cos I was just totally over it. I got the epi and then my mw broke the forewaters (I was still only 4cm at this point, I was 3 when I first arrived at the hospital). After 2 hours the epi was wearing off on one side and I felt pressure in my bum so she decided to check me and I had jumped to 10 cm and he was past the lip and was time to push so no more epi for me I pushed for 2 hours and then started having incredible pain between contractions that actually seemed worse than the contractions, I think because with the contractions I was so concentrated on pushing that they didn't hurt that much Anyway mw got OB to come check my scar in case, it was all good and it was his shoulder causing my pain but it was decided because of that and because how long I had been pushing that we would try the vontouse. One push with the aid of that and his head popped out and then I pushed the rest of him out with the next contraction.

I had been told by the OB at 36 weeks that I would be monitored continuously from the moment I got to the hospital but this did not happen, I was monitored for about 30 mins when I first arrived and had an internal to check dilation, I would have only been monitored for 20 mins but his heart rate was racing because I was dehydrated so I had to drink and wait for his heart rate to come back down. I then wasn't monitored again until I had the epi, the mw just checked now and then with a doppler and asked frequently if he was moving.

I also only saw the OB three times, once when I first arrived, then again at shift changeover and then again for the vontouse. So it doesn't need to be overly medicalised (if that is even a word...lol)

With Bella, I had an epi a couple of hours before my c/s and it was sh*t, it only covered one side and the other side was excrutiating, so you can imagine I was a bit nervous about having another epi with Ben, but...

With Ben I went into spontaneous labour and managed to get to 7cm with just the gas, but I had had contractions 2 mins apart for about 48 hours and was over it so requested an epi. The anaesthetist gave me an epi/spinal combo and it was true bliss! I didn't feel a thing getting to 10cm and my waters spontaneously ruptured while it was in too. Then they let it wear off slightly for the pushing stage - I pushed for 2 hours but still nothing (he was posterior), so had to have the forceps, so was pretty glad it was already in place.

I don't regret it and had a far better labour with Ben (and quicker) than Bella.

NB: I was monitored quite heavily and made to lie on the bed and I think if I'd had the option of being able to move around I would have been able to labour without an epi...

For both of my c-sections I had epidurals well before heading to theatre. #3 where I actually gave birth (finally) I had no intervention at all during my labour.
I think it REALLY does interfere with the natural birthing process, particularly if it is done too soon.

I found other ways to cope with the pain, the best thing having a great support person who made me concentrate on what I was doing & why.

I had CS with DD1. Had epi on Dr advice cos I was getting soo tired in labour as it was long and drawn out. Had ARM and water was green as she had pooed sign of distress. Epi wore off and I tried to push but DD was stuck and ended with Emer cs under GA.

DD2 I decided early on that I wanted Vbac. My CS experience was tramatic and the thought of it scared the shyt out of me even 5 years down the track with DD2. Had long dicussions with OB about this. He was great,put me in touch with a MW who had alot of experience with helping mums have a successful VBAC.

Part of the planning included lots of walking during preg to help strengthen muscles, walking while in labour and being upright if able to to help with gravity and all lol and last was no epi as this had slowed my previous labour not that, that  contributed to CS.

SO here I was walking 2.5km from one end of Petone beach to the other in labour, got stuck on wharf and DH had to retrieve car while I tried hard not to have this baby on the wharf as contractions were over a minute long lol. Got to the hosp and DD2 was born 20mins later standing up, had ARM with her.

I totally agree. Intervention (of any kind) when trying for a vbac is not the best idea.
But they will give you an Epi if you request it while attempting a vbac.

wow yeah I'm with you sally belly, so much to think about. I REALLY wanted a epi for my VBAC cos like you I don't know if I can deal with that much pain again...I had my epi only a few hours before Caden was born by emergency c section, so I laboured a long time and the contractions were so freaking painful, I was starting to loose the plot by the time I got the epi!

I'm going to write a list of questions for the ob when I see him at about 30-35 weeks...

My 1st was an emergency caesar, 2nd first real birth 72 hours of labour from start to finish had ARM about 6 hours before she was born had gas just before pushing, pushed for 15 mins. 3rd homewater birth no pain relief ARM 5 hours before birth another long labour, 4th homewater birth ARM 5 hours before birth only 7 hour labour, 5th had to have her in hopital as she was 2 weeks overdue and had reduced movements. Active labour (I think thats the real painful type I dont really know what they mean when they say that)4 hours. ARM at 1230 started gas (just cause I could) at 100 and had 5 mins pushing born 105. With this bubba pushing for homebirth again.

I have the opposite problem I had a c section with my DD due to her being breech and I had high blood pressure so it was an ugent c section at 37 weeks not emergency, my C section was great was up and showered 5 hours after the birth and only needed voltaren and panadol for pain relief, I was only in hospital longer due to my baby being in the special care baby unit. ALthough I got a cold while in hospital and sneezing after a c-section is not nice!! I have a few years to think about it but I am wondering about having an VBAC for the next one however heard that I would be stuck on the bed and closely monitored, so wondering if it is worth going through that when I had such a good experience with the c section

Perhaps so, Salz, I guess the biggest factor probably would be how old/heavy/independent Samantha is by then.

I spoke to my MW about having an epi the other day. She said that "they" (presumably the hospital staff) like you to have an epi quite early in your labour in case baby needs to be whipped out quickly . I can see their point but...

She also confirmed that I would be closely monitored & therefore wouldn't be able to get in the bath or move around much etc.

So yeah, my birth probably won't be much like I imagined it at all... On one hand, an elective might be "better"
