Hi all - first time poster with a couple of questions for those of you who have done Ovarian Drilling (OD)
We have been TTC for just over 4 years (T1 diabetes, PCOS, minor sperm morphology issues) and are now waiting for OD through Fertility Associates Wellington. We have previously tried Clomid, IVF and IVM.
Questions: 1) How long did you have to wait for your OD surgery? (I've been told to expect up to 5mths) 2) How did you find the procedure, recovery etc? 3) Was it successful for you? Did you start ovulating / get BFP (and if so how soon after)?

Hello My first post here as I saw your message. I would have had to wait 4-5 months on the public system so I phoned my medical insurance. Due to PCOS and not ovulating on my own - I was eligible for private treatment. I was able to have the procedure 2 weeks after I rang them which was on Thursday. I highly recommend private treatment as the care is amazing. It's now Saturday and I am still taking it easy. Sore tummy and pretty tired. from the operation. Every day I feel better. Followup treatment with my Dr is on Wednesday so will let you know how it goes. It has a high success rate with 70 percent of women who have the procedure start to ovulate on their own. Fingers crossed it works

Thank-you so much for your response. Fingers crossed all goes well for you and great to hear that you aren't feeling too bad afterwards. Unfortunately I don't have health insurance (mostly because of my diabetes) so I have to put up with the wait. My specialist through Fertility Associates will do the actual surgery though rather than some faceless stranger which I'm pleased about. I hope your follow-up appt. goes well on Wednesday!

Totally agree about Fertility Associates. Best money we spent was paying for an initial consultation with them (have been on public funding since). Even though we haven't had any success yet (after clomid, 2 cancelled IVF cycles and an unsuccessful IVM cycle) we still feel like they are fantastic with how supportive they are and their management of our treatment.

I had ovarian drilling along with an op for endo in 2009. Mine was covered privately, so there was no wait at all. I found the procedure and the recovery pretty straightforward really. I can't remember how long I had to have off work, but I spent one night in the hospital and a few days feeling a bit tender afterwards. But definitely no agonising pain or anything worth being scared of.
And the good news... I never got a period the month after After a long time trying unsuccessfully, I ovulated a couple of weeks after the op, and we conceived our son. Even better news - since then, I have successfully ovulated on my own each time I got my period back after havig a baby, and we now have three children. The last two were conceived with the same speed as my first, it appears once we actually got my body to ovulate it knew what to do! I hope you have similar success.

What a fantastic result Hopes! Thank-you for sharing. Natural ovulation would be amazing but at the very least I hope it boosts our luck with other treatments. Also great to hear the long-term impact it's had as you read a lot about it being a short-term fix.

Update: Followup appt with my Dr went really well. Everything looks fine from the outside (ovaries, tubes and uterus) - yeah! They could only get dye through one tube and it was perfect. The other tube was inaccessible due to my tilted uterus. So we are presuming that it's is fine too. So all and all a good result. Next steps: Blood tests weekly to see if I ovulate on my own - fingers crossed! If I do then we keep trying for natural conception for 3-6 months. If not, we can then go on to next stage of fertility treatment with injections to provide additional hormones to trigger ovulation. In conclusion, really really pleased I went through with operation and fingers crossed that it works. I will keep you posted.

Update: had a blood test on Monday and progesterone levels are up. I have had period symptoms so will be interesting to see what develops. Another blood test on Monday.

Got my period yesterday. Pretty exciting as this is the first natural period in have had in over 15 years. I went off the pill a year ago and haven't had one since so this is pretty exciting stuff! I can't believe that I can now ovulate naturally!!!

So sorry to hear that regular ovulation hasn't kicked in after your OD. Such a huge disappointment after it was all seeming so positive! Will you be doing anything such as Clomid while you wait for IVF? I've read that even if OD isn't enough to get Ov happening on it's own it can make other methods more effective. I certainly hope so as that's the main goal for me as I've already been down the IVF route with no joy due to poor response. I just got my OD surgery date a few days ago - booked in for end of April.

Hi, thank you for your kindness. I asked my specialist if I could try clomid again as it does make me ovulate but my uterus lining gets thin so not a good medicine for me to take - Buggar ah!
I may ovulate in my own - just not like clockwork.
All the best for your surgery - my fingers are crossed for you. Let me know how you get on or if you have any questions after surgery. I hope you have taken some time off work to recover. I took a week and was so pleased I did. It was nice to take it easy and not have to explain to anyone at work while you felt a little fragile.

Stink about the clomid thinking your lining. Do you use OPKs? I have mixed success with them due to the messed up PCOS hormones but maybe post-surgery they could be useful?
OD scheduled for the 24th which is perfect timing as I had already planned to take those days between Easter and Anzac Day off anyway! Will let you know how it goes.

OPK = Ovulation Predictor Kit. They test the amount of LH in your urine. LH surges just before ovulation so you test for a few days before you think ovulation is due and if you get a positive then you get "busy" for the next 36 hours or so :). Only thing is if you have PCOS a) it can be hard to know when to start testing due to long/irregular cycles and b) some women with PCOS always have high LH so get lots of false positives.
I get some relatively cheap from www.baby4you.co.nz and start use them if I start to suspect ovulation may be on its way through other signs. Because that might only happen once every 3 mths or so if I end up testing for ten days in a row to try and "catch" the surge the cost doesn't add up too much.
They aren't always useful for everyone but they are just another possible "tool" in this battle!

Just an update for anyone who might be researching Ovarian Drilling in the future as I know I struggled to find info when I was waiting.
Had my Ovarian Drilling last Wednesday at Keneperu hospital. Being T1 diabetic I was first on the list which is always great. I checked In at 7.15am and was all done and dusted and on my way home by mid-day (they ask you to bring an overnight bag though just incase). Everybody at the hospital was so lovely and genuine - they really helped keep the pre-op nerves down.
I obviously don't remember anything about the surgery itself but didn't have any issues with the General Anaesthetic wearing off - a tiny bit of nausea (common side effect) which they gave me some meds for (through IV) as soon as I mentioned feeling queasy and they worked almost immediately. So definitely speak up!
The first couple of days I was pretty sleepy but was able to be up and walk around without any real issue. I was pleasantly surprised by how minimal the pain was - easily managed with regular Paracetamol and only really an issue when changing from sitting to standing etc. Vaginal bleeding lasted about 4 days and was pretty light.
I had 3 incisions (sometimes they do 4) with 4-5 "zaps" in each overy. I did get some conflicting advice about wound care with written info saying change the dressings the following day and being verbally told to wait 4-5 days. I went with the latter instruction. I had dissolvable stitches which are meant to disappear after about 7 days and if not they can be removed by GP/nurse. I'm at day 7 now and my belly button stitches haven't quite gone but they are getting there so I'm just going to keep an eye on them for a few more days.
I'm not quite sure what to expect next. I have a follow-up booked with FA at the beginning of June to discuss options but it will be interesting to see what my body decides to do between now and then.
I hope someone finds this helpful/reassuring at some stage. I'd love to hear others experiences.

Hi there thanks for your updates re Ovarian Drilling - I had this procedure done yesterday and have been finding it hard to find recent posts. Would love to hear how you getting on and hope all is well.
I have PCOS and do not get periods naturally at all. I also went straight to Fertility Associates when we started TTC as I knew would have trouble. This was June last year now and since then have been on 4 rounds of clomiphene. The first 3 cycles I ovulated but did not get pregnant. The 4th cycle I ovulated and finally got pregnant but MC after 7 weeks. This was at beginning of March. Plan was to then go back on chlomiphene cycles and my specialist also sent referral for me to have OD through public system as knew there would be a waiting list.
As it turns out I did not get to try chlomiphene cycle again as 2 months after MC still had HCG levels so ended up having a D&C 2 weeks ago. I then got the call up for OD so have gone ahead yesterday.
Feeling ok - quite uncomfortable to move around so just resting up and making sure take Panadol regularly!
Not to sure what to expect from here so would love to hear how you are getting along.

Hi Blair - I really struggled to find anything recent about OD as well so I'm glad you found my posts.
I'm feeling really positive at the moment. I had my OD on CD6 and got my AF exactly 14 days later - I was so stoked! I'm now on CD17 of my first post-OD cycle. I've been using OPKs since CD10. I usually get at least a faint line on every test and a few false positives. But not this time - the test line has been white as snow (so maybe less unnecessary LH floating round?) until this evening when I got a really clear positive! And EWCM! All signs seem to be indicating that ovulation could be right around the corner!
The only downer is that hubby is away at the mo and I'm convinced I'll miss the window this month. Typical! 😝 But after more than 4.5yrs any sign of progress is something to be celebrated!
What area of the country are you in? I hope your recovery goes well and if you have any particular questions let me know!

Wow that is great to hear that you have had such a good response to OD - makes me very hopeful! A shame that you may miss the window this month but like you say after 4.5 years you must be so happy just to have some progress! I have not used OPK's before as have always had monitored cycles through FA - I may think of using them this time round though... Back to work tomorrow feeling a bit more mobile now so hoping should be ok. Will keep you posted with any progress I have over next few weeks - assume will be in touch with FA over next few days to find out next steps. Good luck to you also! (I am based in auckland).

No BBT temp shift this morning and Hubby is home tonight so fingers crossed that a) I ovulate, b) we don't miss the window and c) it results in a BFP!
I recommend Baby4you.co.nz for reasonably priced OPKs if you want to give them a go.
I'm with Wellington FA (though I did do one IVM cycle with Ackl FA as it's not done anywhere else in the country). I have my first post-OD appt with them 3rd of June.
Hope work isn't too strenuous tomorrow! Take it easy if you can and would love to hear your progress!

Hey ladies, I'm interested to read your experiences.. I am hopefully having OD added to my laparoscopy in a couple of weeks, but am not sure what to expect afterwards. Do you wait for AF and then you can start trying after that or?.. I will ring my specialist when I get a chance, I'm hoping I can start on clomiphene again ASAP..

I didn't really get a lot of info about "what next" immediately following surgery. Just got told to make a follow-up appt for 4-6 weeks later. My FA specialist said that we'd see how things were going at that point and decide whether to try a few natural cycles or jump straight back into assisted cycles. My husband has some minor sperm issues as well so even if I'm successfully ovulating I'd rather not muck around too long with completely natural cycles.
With extensive googling I found stories of women who got a BFP straight after OD without even getting a post-surgery AF. To be honest I wasn't really feeling that keen on doing the BD that soon after surgery so was happy to wait till the first fresh cycle.
Do you mind me asking your TTC history beaker?

Bit of a novel.. but I'll try to keep it short!
We started TTC in Sept 2012. My DH works 1 month on 1 month off, so in a year, we had 7 cycles I think, but I went to FA Aug 13 as we were looking at speeding things up with some IUI or something. I had had one long (46day) cycle, but the rest between 30-34. He did a scan and all that was really talked about was that he suspected I had endo, and that I hadn't ovulated that cycle and I was CD26 and already spotting. So he referred me for a laparoscopy. He said it's possible i wasn't ovulating regularly, so I had 3 monitored clomiphene cycles while I waited for my specialist appt for the lap. I also have no fertile mucus in any of my cycles.
They only gave me 25mg of clomiphene, refused to up the dose even though I was only having 16mm or less follicles on CD15/16 scans, because I had "good progesterone results". The hospital specialist suggested i push for a dose increase if I had another cycle. I decided after that 3rd cycle to see a Specialist in my city that I found out did clomiphene (had I known, would have gone there first instead of travel 6hr return!!) where she did a scan and found 14+ cyst/follies in each ovary, significant evidence of endo, once again hadn't ovulated even with a positive OPK and my lining was already coming away at CD21. She was also concerned that my follicles probably couldn't get big enough to ovulate even with the clomiphene in the state they were in. She started me on Metformin which brought my cycle to 28 days right off the bat! I've been slack the last month and this cycle has been longer I think as a result of that. She also wrote to the hospital and asked that they add OD, hysteroscopy and a D&C to my lap on top of the endo stuff and tube flush.
My specialist is happy for me to have clomiphene after surgery if that's what I want, but she didn't give an indication of how long I had to wait. I will hopefully get some info when I call to let them know about my surgery. She wants to do 6 cycles but for me thats a year more TTC, so we have decided we will do 3 and if no success go back to FA and look at either IUI or IVF, whatever is most appropriate from there..
My DH is home for 3 weeks after my surgery, but then won't be home again until Aug/Sept. Hopefully they will do everything that as been requested (just hard since my new specialist isn't the referring Dr) otherwise I will have it done privately..
So yeah. Hopefully this will get the ball rolling!! I hope that FA will put you on the right track, things are sounding really positive for you after surgery :)

What a saga! It's so frustrating when you feel like you're not getting the right treatment plan. We've been trying since 2009. The first year on our own then the second year through the public hospital. We had 8 rounds of clomid - all without any monitoring at all. Little explanation or exploration of other options. It was the worst year! The best thing we did was pay for the initial private consult with FA. Even though we've been with them for over 2 yrs now with no BFP we feel listened to and that they are prepared to try new things all the time.
I travel quite a bit for work (though not as much as your DH!) and know how irritating it is feeling like you have so few chances in the year anyway and then to not be able to take advantage of the rare occasion Ov does happen because you aren't in the same place!

Well, think I may of had my temp shift this morning so time for the dreaded 2ww. We only managed to DTD once (on the evening before temp shift) so not feeling confident about our chances of catching the egg this time round. But getting an egg in the first place is a good start!

Breaker I also didn't have too much in the way of next step instructions before my op but I phoned the nurses at FA yesterday and apparently I will have weekly blood tests starting next Thursday (14 days post op). Once we know whether op has worked or not I guess will then meet with specialist to make a plan I.e just start with natural cycles or clomiphene again. I was like you and eager to start on clomiphene again ASAP but I do understand now that at least need to know if op worked.
Would be so good if by some miracle I get BFP this cycle and don't need to think about next steps again!

Had my post-op follow-up at FA today which went really well. Scan seemed to look like I probably did ovulate (currently 9dpo) and I get the results of bloodtest tomorrow which will hopefully confirm this. If it's positive then we will do a couple of cycles trying naturally. If not (or I guess if they don't work) then we will start on Letrozole. Always feels good to have a plan!

SweetConception, sounds like a great response! I hope it goes well and you don't need the letrozole.
I am having my lap on Monday, I am assuming they've added it as my Surgeon has booked me to stay overnight afterwards, which he said wasn't necessary just for a tube flush and endo removal. So hopefully having Endo removal, tube flush, OD, hysteroscopy and a D&C :/ eek. I have to do a bowel prep, starting Sunday. Not keen!!

Update from me is that I now have post op appointment booked in with FA on 25 June so a while away still. I have still not seen my report following op but it is in the post so should arrive any day and I am excited to see if I can make any sense of it.
Had my first progesterone blood test yesterday and very low levels so no ovulation yet (not that I would expect it) next blood test again in 7 days. According to my Internet researching CD21 is when you woul expect to see high progesterone levels if you have ovulated successfully.
If I count day of op as CD1 the I am now CD 15. I have had no signs of impending ovulation from OPK'S but have had EWCM the last 3 days - do you think that may be a good sign?
Well will let you know as soon as a further update from me but all the best in the meantime girls! :)

Ewcm is definitely a good sign Blair! Fingers crossed you get a positive OPK soon. Or maybe Your first AF is on it's way? I got my AF 2 weeks after surgery and sometimes get CM similar to Ewcm a few days beforehand. Either way it definitely sounds like your body is working hard at something in there!
Yup, CD21 is the standard for Progesterone test based on most females ovulating around CD14. My result (CD27, 9dpo) was a definite positive of 40.2 (I think anything over 30 is considered a sign of Ov).
I'm rapt about the result although nervous it was just a fluke and won't happen again! I'm also feeling a bit down today as I'm pretty sure AF is only a day or 2 away. I knew all along that the chances of getting pregnant this month were tiny but "hope" trumps "logic" every time! Oh well - being on the next cycle!

Great news re ovulation that is step one. Signs of AF can also be signs of BFP but even if not you must be rapt that you are likely to get a chance again next month based on fact that you seem to have responded well to OD.
Does your doctor recommend trying naturally again if no luck this month or straight into assisted cycle?

Since I ovulated this cycle the plan is to try naturally for 2-3 cycles and then review. He has given me a script for Femara though so if this is a fluke and I don't seem to ovulate next month we'll probably start that the following cycle.
Thinking positively I'm doing way less overnight travel next month so hopefully there will be plenty of time to BD rather than the one measly chance we had this cycle! 😉
Are you making the most of the Ewcm at the moment just incase?

Yeah we are - today is 4th day of EWCM (CD day 13-16) and we BD 3 out 4 of those days just in case! Although still no +ve OPK'S and I have now run out.
Going to have to wait until next blood test results which won't be until next Friday now to find out if actually ovulated or not. At least there is a bit of hope!

Hey ladies, they didn't do any drilling for me, apparently my ovaries looked good. I did have a flush and dye though which spilt really well, and had a polyp removed from my uterus and endo removed from behind it. I have the week off work, had to stay overnight and got home just before lunch. I am starting to feel better getting up, but the shoulder tip pain can be killer!

Glad it all went well Beaker and that you aren't feeling too bad. Awesome that your ovaries looked good and they didn't need to zap them! Re. The shoulder pain - I was told to put a pillow under my hips. I don't know how much difference that made but I ended up not having much pain so maybe it did the trick!

Glad you feeling ok and good news that no drilling required. The shoulder pain was such a surprise to me - was last thing I expected to be sore after op!
Is good that you have week off work to recover.
I am also home in bed - have a really bad kidney infection. I got it initially straight after operation (so make sure you keep drinking lots of fluids to flush out any infection!). I went on 5 day course of antibiotics but has come back with a vengence so now on 10 day course.

Hi ladies
Good day for me today - first day up and out of bed feeling so much better after kidney infection AND had day 21 blood test which shows that I did ovulate!!!! Wahoo first good news in soooo long :)
The nurse said was probably about a week ago (I think it was Saturday). So then will have another day 21 test next month to make sure wasn't a fluke. My follow up appoint with specialist is in a couple of weeks so guess we will talk then about what next steps but I imagine same as you SWEETCONCEPTION that will just try 2-3 cycles naturally and see what happens.
Did AF arrive for you?

Well we certainly tried and BD over the right time so I guess will know any day now. Thing is I think I feel like AF is very close :( will be disappointed but no point worrying about it too much until I know for sure.

Fingers crossed for you Blair - when will you test?
I'm cd9 on second post-OD cycle. CM has just changed from dry to sticky/creamy so hopefully a good sign that last cycle wasn't a fluke and that I'm gearing up for ovulation. I ov'd cd18 last cycle so might be a week away yet but will start OPKs tomorrow.
Ready for it to be my "turn"!

Well today is 10 or 11 DPO (well at least from when I think ov happened as OPK'S were not working and only confirmed by blood test a week later) so I am thinking if AF does not arrive by Saturday/Sunday then I will test or do you think I could do sooner?
Yes I know what you mean very ready for it to be "my turn"! That is sounding positive that you are looking like may have another chance again very soon.

Definitely hold out testing for as long as you can - preferably till 14 dpo. But I never follow my own advice! 😄

So an update - I manged to hold out to 14dpo before testing and did two home tests which were both positive and then a blood test today to confirm BFP!
I am too anxious to be excited after MC last time but very happy that step in right direction - it is my birthday as well so what a birthday present!

I'm at Cd20 and no sign of Ov this cycle 😕. Feeling pretty gutted. And angry at myself for getting my hopes up that. OD would be my magic bullet after such a long journey. So moping around a bit at the mo and trying not to be too much of a grumpy-bum! 😜 Hoping cycle will end quickly so I can "move on" again. Will take Letrozole next cycle and see if that helps.
How are you doing? Told many people yet that you got your BFP or keeping it under wraps for now? Will you be getting early scans?

Oh that is crappy sorry to hear no sign yet - mind you weren't you CD 18 last time? Possible that just taking a while again?
Will you go back to doctor before next cycle or had you already made a plan of how to proceed? I know what you mean either way just want cyce to hurry up so can start with the next option. The waiting in between is the worst. Fair enough to be mopey!
Have only told parents re BFP noone else yet and to be honest still haven't really accepted it myself yet as dont want to get excited and then feel disappointment like last time again - too painful.
Had BT on Friday that showed hcg levels rising which is good - will have another BT again in a week and then early scan (6-7 weeks) the week after. It was at that early scan last time that we found out things were not ok so once we over that mark hopefully I will let myself enjoy it a bit more!

Hey SweetConception, if you don't mind me asking, what prompted them to start you on letrozole, and does it need less monitoring? My clomid was only half the usual dose, but seems to have had an effect on my lining in cycles after it, and I also have very little fertile mucus. I'm thinking I might ask FA if they'd prescribe me the letrozole instead of trying clomid again. I have read there is also a lower risk of multiples?

Thanks Blair. Yeah, Cd18 last cycle. I guess I feel like late ovulation reduces the chance of success. I've heard that the lining gets old and makes implantation less likely. Who knows?
I can understand your hesitance at the moment. It must be nerve-wrecking! Good to hear your results are rising. Hope your next tests/scans continue to go well.
Beaker - I've done loads of clomid cycles over the years - all except a couple were unmonitored so I don't really know what effect they had. The couple that were tracked I had low/no response to (which is what happened with IVF too). So I'm going to try Letrozole to see if I have a better response. I asked about multiples and got the impression that the risk levels weren't that different. I'm not sure about other side effects but it definitely seems like it would be worth chatting to your specialist about if you had such a reaction to a low dose.

I don't have much mucus to start with, it gave me a little more, but my cycles were still longer and stuff. They were all monitored but very low response so they didn't increase it. I am trying to be much more assertive with my follow up, but I don't really want any monitored cycles at the moment, as I need a break from all the medical stuff. Hopefully between my GP, FA and the specialist I saw here, someone will help me get there without too much intervention. Thanks for the info :) Good luck with the letrozole :)

Just a quick update as I know it can be frustrating when you find a thread about a specific topic (e.g. ovarian drilling) and it just dries up...
Unfortunately the ovarian drilling doesn't seem to have done much good for me. Since the OD in April this is what has happened with my cycles...
Cycle 1: natural ovulation cd18 confirmed through OPK, temp and BT - yay! Cycle 2: no ovulation, AF induced with Provera Cycle 3: 2.5mg Femara (unmonitored). Possible Ov. Cd35 - way too late Cycle 4: 5mg. Femara (monitored) with plans to do trigger shot. Cd10 Estrogen too low and Cd14 scan showed approx. 60 follies all under 10mm! Cycle cancelled. Currently on Cd25.
I absolutely don't regret getting the OD done in any way but we are obviously disappointed it hasn't given the results we were hoping for. For the first time in 5yrs our FA specialist has said he's "starting to run out of ideas" to conceive using my eggs. We've also used up all of our public funding for cycles with my eggs (some left if we swap to donor eggs). Current plan is to have one last shot at privately funded IVF with my eggs before investigating NZ (funded) or international (private) donor egg options.
